Field Trip Request Contract
Thank you for choosing the St. George Children's Museum for your upcoming field trip!
Field trips can be scheduled on Mondays. The minimum number of guests is 50 with a maximum of 150. Field trips MUST have a 1:5 adult to children ratio. If you are looking for an alternative option, After Hours events, which are held after 5 p.m. and include exclusive use of the Museum- are also available for reservation. Please call the Museum with any questions at (435) 986-4000.
Complete the following form to the best of your knowledge. Any missing or incomplete information may affect the field trip scheduling.
FIELD TRIP REQUEST FORMS ARE NOT CONFIRMATIONS.
Please allow 48 hours, or 2 business days, for the field trip request form to be reviewed and preapproved. Please wait for a phone call or email from the SGCM Guest Services Coordinator to confirm. Please also note that the SGCM must receive all field trip requests TWO WEEKS prior to the requested date. Any requests that do not follow this rule will not be accepted.

For school funded educational trips a fee of $4.50 per student applies; chaperones are free and any siblings 3 years old and above that plan on attending with the field trip students, are $8.50 each.

Please allow 48 hours (2 business days) for the request form to be reviewed and approved. Based on the answers submitted, you may be asked to provide additional documentation. Failure to provide the additional documentation, or completing the final steps to scheduling the field trip may result in a cancellation. Museum Waiver:
In consideration of the services of the St. George Children’s Museum (“SGCM”), their agents, owners, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf, I hereby agree to release, indemnify, and discharge SGCM and all associated individuals and entities, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows:
1. I acknowledge that my participation in activities at SGCM entails known, unknown, and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.
The risks include, among other things: slips and falls; falling from equipment; sharp corners on exhibits, the building, or equipment; small exhibit pieces and other choking hazards; stairs and other changes in elevation in the building and on exhibits; rope burns; pinches, scrapes, twists and jolts that could result in scratches, bruises, sprains, lacerations, fractures, concussions, or even more severe life threatening hazards; strains, cuts, bruises, muscle soreness and fractures; musculoskeletal injuries including head, neck, and back; injuries to internal organs; crowded spaces; dark rooms, flashing lights, and other light-sensitive areas which could be harmful to people with epileptic conditions or vision impairments; movable exhibits; live animals; ridable vehicles; the negligence of other people; the negligence of other participants, or other persons who may be present; my own physical condition; and the risk of emotional and psychological injuries or physical damage associated with this activity. In any event, if your child is injured, your child may require medical assistance, and you agree that this will be at your own expense and not the responsibility of SGCM.
I acknowledge that, despite listing some potential dangers above, I am not relying on SGCM to provide a full and exhaustive list of potential risks at SGCM. I recognize that there are potentially other unstated risks and agree to assume them as stated herein as a condition of being granted access to SGCM.
2. I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary, and I elect to participate despite the risks.
3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless SGCM from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of SGCM’s equipment or facilities, including any such claims which allege negligent acts or omissions by SGCM.
4. Should SGCM or anyone acting on their behalf, be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs, and such costs shall be recoverable in any such action.
5. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.
6. In the event that I file a lawsuit against SGCM, I agree to do so solely in the state of Utah, and I further agree that the substantive law of that state shall apply in that action without regard to the conflict of law rules of that state. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining document shall remain in full force and effect.
